چکیده

Myocardial ischemia is well established as a reliable predictor of physiologically significant coronary artery disease (CAD) (1,2). During the past two decades, a large number of epidemiologic studies and prospective observational data have demonstrated that the presence of myocardial ischemia is associated with adverse outcomes in asymptomatic individuals as well as those with established CAD (3–22). The results of these reports emphasizing the prognostic significance of myocardial ischemia in asymptomatic individuals have led the American Heart Association and the American College of Sports Medicine to recommend that apparently healthy men ages $45 years and women .55 years (even those without known cardiovascular risk factors) should undergo a maximal exercise treadmill test before engaging in a vigorous exercise program (23). However, the issue of routine screening of apparently healthy individuals for myocardial ischemia continues to be a controversial issue and a topic of constant debate. This issue is further compounded by the fact that, frequently, myocardial ischemia detected by ST-segment depression during stress testing in healthy individuals is not associated with chest pain. Because of the absence of symptoms, it has been suggested that the ST-segment changes without asso-

Exercise-induced silent myocardial ischemia and coronary morbidity and mortality in middle-aged men.

OBJECTIVES We investigated the prognostic significance of exercise-induced silent myocardial ischemia in both high and low risk men with no prior coronary heart disease (CHD). BACKGROUND Silent ischemia predicts future coronary events in patients with CHD, but there is little evidence of its prognostic significance in subjects free of CHD. METHODS We investigated the association of silent i...

Synergistic Effects of Depression and Poor Impulse Control on Physical Partner Violence; A National Longitudinal Study in United States

Background: Although major depressive disorder (MDD) also increases the risk of Intimate Partner Violence (IPV) perpetration, poor mental health has been mostly conceptualized as a consequence of IPV victimization rather than a risk factor for IPV perpetration. Poor impulse control is another IPV risk factor. Building on a risk / risk framework, this study investigates additive and multiplicati...

Association of exercise-induced, silent ST-segment depression with the risk of stroke and cardiovascular diseases in men.

BACKGROUND AND PURPOSE There are few if any data on the prognostic importance of silent myocardial ischemia during exercise with regard to the risk of stroke and cardiovascular diseases (CVDs) among asymptomatic men. In this prospective study, we investigated the relation of silent myocardial ischemia and the risk of stroke and CVD death in men with and without conventional risk factors. METH...

Silent subcortical brain infarction

Introduction: The silent brain lesions detected by MRI were fairly common not only in first-ever stroke but also in normal elderly subjects. Some recent studies show the possible role of silent sub-cortical brain infarction in ischemic stroke. The aim of this study was to evaluate the frequency of silent sub-cortical brain infarction in acute first-ever ischemic stroke. Methods: In this descrip...

Scottish foot ulcer risk score predicts foot ulcer healing in a regional specialist foot clinic.

OBJECTIVE To determine whether patients developing ulcers had previously been identified as being at high risk of ulceration using the Scottish Care Information-Diabetes Collaboration (SCI-DC) ulcer risk score and whether the risk score predicts ulcer healing. RESEARCH DESIGN AND METHODS All patients attending the diabetes foot clinic with an ulcer had been assessed for foot ulcer risk using ...
